What is Home Health Care?

Home Health care is care services in your home for an illness or injury. Is less expensive and convenient. Examples of skilled needs are: Wound care, Patient and caregiver education, Intravenous or alternative nutrition needs, Monitoring serious illness and unstable health status. The Goal is to get better, regain your independence, and slow decline. Your Medicare health plan will cover needed care, and your Dr’s office can give you a list of agencies that service your area, they must tell you whether their organization has a financial
interest in the agency listed. You will need a Doctor’s orders for Home Health Services. The Agency will come to your home to provide an assessment and review the benefit.

What is Hospice?

Hospice is quality and compassionate care at home, for people facing a lifelimiting illness or injury. It’s medical care toward a different goal: of maintaining or improving quality of life for someone whose illness, disease or condition is unlikely to be cured, when curative measures have been exhausted and life prognosis is six months or less, but can exceed this time period. Hospice care involves a team-oriented approach to expert medical care, pain management, and emotional and spiritual support, expressly tailored to the patient’s needs and wishes. Support is provided to the patient’s loved ones as well Hospice focuses on symptom management.

Care is provided in the patient’s home Hospice services are available to patients of any age, religion, race, or illness Hospice care is covered under Medicare, Medicaid, most private insurance plans, and HMOs


Medical eligibility

In order to receive hospice services, a hospice physician and a second physician (often the individual’s attending physician or specialist) must certify that the patient meets specific medical eligibility criteria. If the disease or condition runs a continued declining course and life expectancy is 6 months or less, this meets criteria. When the individual lives longer than six months and their condition continue to decline, they may be Recertified by a physician for additional time in hospice care. If a hospice patient’s condition improves, they may be discharged from hospice care, or they may be discharged from the program. But The patient is eligible for hospice again if his or her condition begins to decline again.

Medicare patient’s Have the right to choose any Hospice company and Can transfer to another Hospice company if not satisfied with current hospice providing the service.

Family Caregiver Support Program

Supportive Services Program
The Older Americans Act (OAA) can help to provide Supportive Service Programs to enables older adults access to services that address functional limitations, promote socialization, continued health and independence, and protect elder rights. Maybe able to help provide funding for Personal Care, Homemaker, and Chore programs, to provide assistance for individuals and help them remain in their homes. As well as Adult Day Care, Case Management, Assisted Transportation, or vouchers, and Legal Assistance.
